Testimony and Photo Release Form

I, ______________________________, understand the testimonial I have provided or am providing to Rami Michelle Coaching was created at the request of and on behalf of Rami Michelle Coaching and may be published, distributed, and used in connection with promoting Rami Michelle
Coaching’s coaching services and programs.  I hereby irrevocably authorize Rami Michelle Coaching as well as its affiliates, employees, agents, assignors, transferees, licensees, or any other individual or party designated by Rami Michelle Coaching the unlimited, irrevocable right to copy, publish, distribute, edit, and use in commerce the testimony I provided on  ________________(date) in reference to Rami Michelle Coaching’s coaching services for the purpose of publicizing it’s coaching services, Rami Michelle Coaching, or for any other lawful purpose. The testimony may be used on websites or by related affiliates, printed publications, other presentations, or in any other distribution media. I agree that I will not seek any payment or make any claim against Rami Michelle Coaching for use of my testimony. I also waive any right to approve the finished testimony wherein my photograph, likeness, or words appear.
 

I also authorize Rami Michelle Coaching to use, publish, and otherwise distribute my photograph, name, and city and state where I reside, all of which I provided voluntarily to Rami Michelle Coaching for use. I agree that Rami Michelle Coaching may use this photograph for any lawful purposes, such as publicity, illustration, advertising, and website content.
 

I hereby hold harmless and release Rami Michelle Coaching from any causes of action, claims, or demands which I or anyone else acting on my behalf of on my place may have by way of this authorization.
